Our training programmes have a strong focus on the human element in complex operations. Moreover, all trainings feature details of true events and incidents. This is the best way for course participants to identify themselves with situations they may find themselves in one day. Incident investigations are therefore a vital aspect that continuously supports the factual accuracy of our trainings and services. 

We conduct an incident investigation as a structured fact-finding process to identify the root cause of accidents, injuries and near misses in the workplace. We establish exactly what happened and why, empowering our clients to implement systemic corrective actions that prevent future occurrences, rather than assigning blame. 

Depending on the severity, the regulatory and legal implications, we perform an ACA (apparent cause analysis) or an RCA (Root Cause Analysis). As a result of every investigation, we assist our client in establishing a Risk Based Process Safety (RBPS) framework designed to manage operational hazards and risks. 

Lessons learned and recommendations will drive the continuous quality improvement process, develop the safety culture and improve the reality level of the trainings we provide.
